Our #FPOW this week is the fantastic Dementia Support group in #WestSussex
 
Founded in 2014, they are a young and ambitious charity. The dream was to bring together the different dementia services that existed in the local area into one dementia-friendly community hub; and after an extremely successful capital appeal, that’s exactly what they did.
 
Their objective remains to work alongside existing organisations to bring together and enrich their offering and the services they provide to ultimately provide exceptional #DementiaCare in the #WestSussex area.
 
Their goal has been to develop a definitive integrated model in continuing care for people living with #Dementia, as well as providing full support for families and #Carers.
